    Alameda County Health Coach Program is a Community Health Worker on-the-job training program for young adults to gain skills and access to vertical health careers while improving access, self-empowerment and self-management for patients with chronic diseases in underserved communities. ACHCP provides an important link and opportunity for all participants with training and education to enter the workforce towards thriving careers in the health industry.

    ACHCP aims to improve the health of community members. Community Health Worker is an innovative method to improve health outcomes by motivating patients to manage their health. CHWs assist patients in understanding their health plan, making medical appointments, identifying resources and advocating for themselves. The Program help bridge the gap between clinician and patient by helping patients navigate the health care system, offering emotional support, and serving as a continuity figure. CHWs are trained in chronic disease management, motivational coaching, patient advocacy, and healthcare system navigation.

    The CHW trainee, a non-benefited 6-month position, is headquartered in San Leandro, CA and will work under the direction of the Health Coach Program Director. The trainee is eligible to enroll in community college courses offered at Chabot College to obtain a Community Health Worker certificate. The community college courses are available at no cost to the trainee. Trainees will have 18 months to complete CHW coursework.

    This is a wonderful opportunity for a person interested in supporting efforts aimed at improving the health outcomes of their community and looking to gain experience in residential outreach, recruitment, case management, and health-related work. The successful candidate will have an interest in and enthusiasm for addressing the health disparities of underserved communities.

    Scheduled Weekly Hours: 20
    Compensation: $1,800/month (6 months)
    Paid Holiday and Sick Time: 5 sick days and 3 vacation days

    Additional Information: Trainees are strongly encouraged to enroll in community college courses but are not required to.

    Additional CHW Certification Information: Trainees are encouraged to enroll in coursework at Chabot College to obtain certification as a Community Health Worker (CHW). The Health Coach Program will cover the course fees for trainees. These courses are encouraged but not required.

    The Health Coach curriculum counts towards CHW certification. If trainees complete 3 of 4 courses at Chabot College, they will be eligible for CHW certification. The courses are as follows: (1) Introduction to Public Health 2) Medical Terminology & The Human Body 3) Introduction to Social Work and Human Services 4) Introduction to Psychology-Counseling in a Multicultural Environment.

    Trainees are eligible to complete community college courses towards CHW certification for up to 18 months after the program commences.

    Chabot College offers a Medical Terminology course for Fall 2024

    Minimum Qualifications
    • Must be 18 years-old or older
    • High school diploma or GED
    • Excellent relationship building skills
    • Excellent verbal and written communication skills
    • Experience working with or coordinating various organizations
    • Ability to work with a diverse group of individuals (including age, gender, race/ethnicity, etc)
    • Ability to take direction and work independently on assigned projects
